Who We Serve

We provide services to adults (18+) who:

Are enrolled in a County Mental Health Plan (MHP)
Receive Full-Service Partnership (FSP) services
Require 24-hour structured support to remain safely in the community
Have a history of: Psychiatric hospitalizations. Housing instability. Crisis episodes or high service needs.

Activities of Daily Living (ADL) Support

We help residents build independence in: Personal hygiene and self-care. Meal preparation and nutrition. Housekeeping and organization. Medication management. Financial and daily living skills

Mental Health & Behavioral Support

Implementation of individualized care plans. Behavioral intervention and de-escalation. Relapse prevention strategies. Emotional regulation support

Individual & Group Programming

Daily structured activity calendar. Life skills and social skills groups. In-home and community-based activities. Meaningful engagement opportunities

Community Integration

Transportation To Medical and mental health appointments Community outings and activities Support navigating public settings Development of independence in the community

Care Coordination & Team Collaboration

We work closely with: County Intensive Placement Teams (IPT). FSP providers and clinicians. Conservators and families.

Services include Monthly treatment team meetings. Ongoing progress updates. Immediate communication on behavioral or medical changes

Linkage to Essential Services

Primary care and medical providers. Substance use treatment programs. Social services (SSI, GA, Medi-Cal). Housing and long-term placement resources.

Individualized Care Planning

Each resident receives: Comprehensive ADL Needs Assessment. A personalized Appraisal Needs & Services Plan. Measurable goals focused on independence. Ongoing updates and progress tracking.

Residents are actively involved in their care planning and recovery journey.
