FACILITY TYPE

 Crosspoint Residentials are  Adult Residential Facilities (ARF) licensed by the State of CA, Community Care Licensing management Division (CCLD). These facilities are owned by Crosspoint Residential LLC and is under the management of Patrick Ofose, an ARF Administrator and a seasoned service provider for clients with mental health challenges.

We provide services for adult males and females with diagnosed psychiatric conditions. We are located in Sacramento County.

Our homes serves a total of six (6) clients, all ambulatory in each of them, except Staypoint Residential which is a mixture of Ambulatory and Non-Ambulatory. Crosspoint Residentials provides 24 -hours. per day; 7 - d


We provide services for adult males and females with diagnosed psychiatric conditions. We are located in Sacramento County.

Our homes serves a total of six (6) clients, all ambulatory in each of them, except Staypoint Residential which is a mixture of Ambulatory and Non-Ambulatory. Crosspoint Residentials provides 24 -hours. per day; 7 - days per week care, supervision, support and mentoring for clients with psychiatric conditions - schizophrenia, bipolar disorder, depression and other diagnosis affecting client's ability to function in their day to day activities.

Crosspoint Residentials offers a safe, consistent and nurturing environment specific to clients with limitations in their daily living skills. Our direct care staffs and the Administrators work together to assist clients with Activities of Daily Living (ADL) and guide them to acquire necessary skills towards a more independent living. Staff members are sensitive to clients various diagnosis each clients is seen as a unique individual with their own strengths and weaknesses. All care staff, in coordination with other social service professionals are skilled in preventing, identifying and managing challenging behaviors related to psychiatric problems. Clients are provided education and support in managing their medications.

 Our homes utilizes a person-centered approach - this means ensuring that each client is at the center of decisions which relate to his or her life. The person is the decision-maker. Staff listens, plans together with clients, coaches, shared ideas, and seeks feedback from the clients regarding the care they receive from the home. Clients

Clients are given choices - persons to interact with, activities, how to budget get their money, food choices, things around them that express his or her individuality, etc. and these choices are respected. All of these are provided in the comfort of a place where they can feel safe.
